Informing students:

In agreeing to this  information I declare that I have read and understood that I have access to relevant policies and declare that I have been informed of the following:

1. The code, title and currency of the training product to into which I am being enrolled, as published on the National Register.
2. The training and assessment, and related educational and support services THRSC will provide to the me including the:
   1. estimated duration
   2. expected locations at which it will be provided
   3. expected modes of delivery
   4. name and contact details of any third party that will provide training and/or assessment, and related educational and support services to me on the RTO’s behalf, and
   5. any work placement arrangements.
3. THRSC’s obligations to the student, including that THRSC is responsible for the quality of the training and assessment in compliance with the Standards, and for the issuance of the AQF certification documentation.
4. My rights, including:
   1. details of the RTO’s complaints and appeals process required by Standard 6, and
   2. if the RTO, or a third party delivering training and assessment on its behalf, closes or ceases to deliver any part of the training product that the I am enrolled in.

1. All relevant fee information including:
   • fees that must be paid to the RTO, and
   • payment terms and conditions including deposits and refunds
   • the learner’s rights as a consumer
   • the learner’s right to obtain a refund for services not provided by the RTO in the event the:
     1. arrangement is terminated early, or
     2. the RTO fails to provide the agreed services.

Training and assessment by a Third Party:

I am aware of the following:

1. That I will be informed where the training and assessment course that I will be undertaking is being delivered by a Third Party on behalf of THRSC.
2. I can make contact directly with THRSC at any time via phone: 02 9630 6311 or email thrsc@redeemer.org.au
3. I can lodge a complaint or appeal or a complaint about a Third Party directly with THRSC via the Formal Complaint or Appeal Form.

Student declaration

In agreeing to this declaratation I confirm that I have been informed as outlined in section 2 above and will abide by with my obligations as follows:

1. any requirements the RTO requires the learner to meet to enter and successfully complete their chosen training product, and
2. any materials and equipment that the learner must provide, and
3. my conduct will enable the successful facilitation of the course and will not jeopardise the learning outcomes for myself or other trainees, and
4. I will abide by all policies and procedures as specified by THRSC.
